The coding environment is constantly changing. The Centers for Medicaid & Medicare Services (CMS), the Office of the Inspector General (OIG), and Medicare Assigned Contractors (MACs) are dictating and enforcing their rules, increasing over-sight. Documentation and coding guidelines are not taught in medical schools. Most documentation and coding knowledge is gained during a provider’s years as a resident. The problem is if the teaching physician’s knowledge is limited, so too will the resident’s.
